This Managed Care-Voluntary Plan is set up similarly to the Managed Care Plan, except that the employee pays the monthly rate. This plan works well for companies where only some of the employees wish to have vision benefits. Then just those that voluntarily want coverage need to pay the monthly rate.
The monthly rates are based on single, couple, and family. One and two year plans are available. This type of coverage entitles the employee to receive an exam, frame and lenses. Co-payments are eliminated within the plan guidelines.
